In-Home Services

In-Home Services offer training, support, and personalized care to families and caregivers who have children or adults with a broad range of abilities and needs living in their homes or in foster care settings. We support families in caring for family members so that they are able to remain in their family homes.

These services are designed to support each family’s routines, values, and aspirations for their loved ones. We will assist individuals with a variety of services at home and in the community as designated by the individual and their family. Those services include: daily living routines and independent living skills (personal hygiene, money management, transportation training, domestic tasks, social skills, problem solving skills, etc.), community inclusion activities, recreation, and leisure interests.

Individualized Housing Options & Services

Individualized Housing Options (IHO) are developed to serve individuals in their own living environment. We will provide training, support, and personalized care to individuals with a broad range of abilities and needs as they develop to their fullest potential. We will assist individuals with a variety of services including:

Independent Living Skills (ILS)

Developing and maintaining the skills necessary to maintain their independence in the community. Services may include assistance, training, or supervision in the areas of self-care, community living, communication skills, or socialization.

Supported Living Services (SLS)

Acquiring, maintaining, or improving skill areas such as: communication & interpersonal skills, community participation, leisure & recreation, household chores, meal preparation, personal grooming & cleanliness, money management, reduction of maladaptive behaviors, and sensory & motor development.

Personal Support

Non-medical care, supervision, and assistance in the home of the individual or in the community to achieve increased independence, productivity, and inclusion in the community.

Companion Services

Non-medical care, assistance, supervision, and socialization in accordance with a therapeutic goal in the plan of care.

24-Hour Emergency Support

On-call counseling and problem solving due to health or personal emergency.

Supported Employment Services

Finding and maintaining employment by identifying and assessing employment opportunities and preparing the individual for all aspects of potential employment. Services may include on-the-job training and monitoring, career counseling and support, and developing outcomes to increase an individual’s value to their current or potential employer.
