Current Living Opportunity-Brooklyn Park
Homeward Bound currently has two openings in our Intermediate Care Facility (ICF/DD). This home serves 32 individuals and is comprised of four homes serving eight individuals in each home. Both openings have private bedrooms.
This home provides 24 hour nursing services to children and adults with higher medical needs with various physical and intellectual abilities. Each home has a fulltime House Supervisor and Nurse Case Manager. The staff to individual ratio is 1:2 as well as support staff and awake overnight staff.
This home also provides Services During the Day for individuals who have high medical needs and are unable to attend on off site Day Treatment/Habilitation Program.
